ActivCare
ActivCare - is a web, mobile, and CRM platform designed to streamline palliative care by centralizing communication and patient management. The platform enables healthcare teams to monitor patients, manage shift handovers, coordinate with families, handle payments, and maintain end-of-life documentation, all within a single, connected system. By replacing fragmented workflows with a unified experience, ActivCare improves care coordination, reduces communication gaps, and supports better outcomes for patients and their families.

As the Lead UX Designer, I directed the UX strategy for a cross-platform healthcare solution spanning web, mobile, and CRM applications. I coordinated the work of dedicated web and mobile designers while collaborating with the Project Manager, developers, and stakeholders to deliver a consistent, user-centered experience across all touchpoints.
Responsibilities
Defined the end-to-end UX strategy across web, mobile, and CRM.
Facilitated stakeholder workshops and requirement gathering.
Conducted user research and synthesized findings into UX recommendations.
Developed information architecture, user flows, and interaction models.
Reviewed and aligned deliverables from the web and mobile designers.
Built prototypes to validate workflows and interactions.
Worked with developers to ensure design quality during implementation.
Led usability testing and incorporated findings into future iterations.

PROBLEM STATEMENT - Palliative care involves multiple stakeholders working around the clock.
Current challenges include:
Time consuming patient endorsements during doctor shift changes
Manual nursing documentation
Families lacking real-time visibility into patient conditions
Payment processes handled outside the care workflow
Difficult retrieval of patient records after death
Administrative burden in preparing legal and government documents
These inefficiencies increase operational workload while creating stress for both medical teams and families.
